RAD MEMBERSHIP APPLICATION...

Please note: the name you register as a RAD member will be used with your UK registration number and on all communication, RAD ID Badges, and efforts to support you under the Equality Act 2010.

RAD trains Assistance Dogs for UK MHDI (Mental Health Disability Impairment) handlers only.


RAD requires proof of your long-term mental health illness. You must be able to provide proof of a MHDI in order to become a RAD member.

Have you got your documentation to hand?

- Evidence of your Mental Health Disability.

- Vet health and vaccination evidence

- Dog insurance certificate


Once this form is completed, you will be forwarded to our behaviour  questionnaire.

Once we have received your application form and behaviour questionnaire, we will get back to you to arrange for a zoom induction training session.
